Photography Consent
I grant permission for ProBound Training to photograph my child during activities, and I understand these photos may be used, without restriction, for promotional materials, social media, or related purposes.

Liability Waiver
I, the parent/guardian of the minor named herewith, consent to participation in ProBound Training programs. I release and hold harmless ProBound Training, its successors, employees, agents, and officers from any and all claims, demands, or damages arising from participation, including personal injuries, property damage, or medical treatment. I agree to indemnify ProBound Training and its representatives for any costs, including attorney’s fees, resulting from the minor’s intentional or grossly negligent actions during participation. I confirm that she/he is in proper physical condition and will provide required equipment for participation. In case of emergency requiring medical attention beyond first aid, I authorize a physician or hospital designated by ProBound Training to provide care.
